Preparing for your appointment

You can use these identity documents:

• A valid passport. 
• A valid identity card (ID card).
• A valid driving licence.
• A valid moped licence (AM).

• A valid W card for asylum seekers.

• A valid alien registration document (residence document).
• A valid Dutch alien registration document (visa).

Have you moved house, or have you switched to a new insurance provider or GP? You can update your personal information at any desk at the hospital.

We want to keep MRSA bacteria out of our hospital. MRSA (methicillin-resistant Staphylococcus aureus) is usually found in places where a lot of antibiotics are used. This includes hospitals (especially outside the Netherlands) and large cattle farms.

We need to make sure that you don’t carry these MRSA bacteria. If the bacteria spread to other people, this can be a problem. That’s why we ask you some questions when you visit the outpatient clinic or when we admit you to the hospital.

Please tell us if:

• • You know that you have MRSA.
  • You live or work on a cattle farm.
  • You have been admitted to hospital outside the Netherlands in the past 6 months.
